Design of Care Services to Succeed in Risk-Based Business Models

What We Did

  • Engaged by the COO
  • Collaborated with project team of subject matter experts
  • Designed a new integrated care model
  • Created an implementation checklist
The
Challenge

Value-based healthcare companies are in an ideal position to pilot new models of care. So why is it that so many are incapable of thinking past the front door of the emergency department? For this reason, our client sought excellence and wanted to be ahead of the curve when seeking new operational approaches to managing care.

Our client is physician-led, clinically-focused and supported by a robust, yet agile team of subject matter experts from medicine, nursing, business, medical documentation, analytics and informatics. The challenge that the team faced was to uncover and employ ideas that enable primary care physicians to provide the finest possible care.

Benjamin & Bond’s facilitation and input helped the collaborative group create the specification for a new professional provider entity, evolve their “transitions of care” team, design a specialist services group and establish a mission control team.

The
Solution

Benjamin & Bond was engaged to produce and facilitate a workshop that kickstarted the redesign of its care programs. Expert Benjamin & Bond strategists divided the two-day workshop into four collaborative sessions that helped the client teams establish a vision for the next generation of their care programs, generate specific operational ideas and outline implementation considerations.

Following the workshop, Benjamin & Bond distilled all of the operational changes into an actionable step-by-step checklist to help guide change management and implementation.

Benjamin & Bond’s facilitation and input helped the collaborative group create the specifications for a new professional provider entity, evolve their “transitions of care” team, design a specialist services group and establish a mission control team. Together, we created user experience parameters for new software for the mission control team.
